What to Expect at Your First Appointment:
Your first visit will cover the critical elements of a)information gathering, b)health assessment and c)developing an action plan and recommendations. To do this we need you to fill out our initial intake forms and bring them with you on your first visit. Please take your time for filling out these questionaires, since the information they can provide to us, can be very crucial in identifying the root causes of your health issues. Please try to arrive 15 to 20 minutes earlier to allow our staff enough time to finish your paperwork prior to your appointment time. We also ask that you bring with you the following:
1) Health history from childhood onward including sicknesses, hospitalizations, accidents, surgeries etc. in a chronological order. This can be done from your own memory and not necessarily from your doctor’s files.
2) A complete list of all physical symptoms like headaches, digestive problems, aches, and pains, etc. Please ensure that you do not omit any symptoms. Major and minor symptoms are all important.
3) If you are taking any medicines or supplements please bring the bottles with you.
Our initial check-up takes one to two hours on the average, and the cost of the check-up is $195.00 plus tax. This does not include the cost of natural remedies. Follow-up visits are approximately 30 minutes and cost $63.00 plus tax. For your convenience, we accept cash, debit card, VISA, and MasterCard.
